CLCH Brent Diabetes Service aims to deliver high-quality specialist diabetes care in the community and closer to the home of patients. It is a consultant-led service, which offers specialist clinics on Tuesdays at Willesden and Friday at Wembley site and supports the borough's 52 GP practices/ Primary Care Networks in providing enhanced diabetes care.
The service's main focus is to strengthen and increase the overall management of Type 2 diabetes by offering accessible services that improve the health of patients.
What the service offers:
• a named link diabetes specialist nurse per Primary Care Networks.
• Multi-disciplinary diabetes clinics in the community
• Direct email / telephone support for GPs, practice staff and patients
• diabetes 'virtual' clinics
• Diabetes education and self-management for ongoing and newly diagnosed (DESMOND) Online over 2 days (3hrs) and Face to Face (6 hrs)
• Group Injectable sessions
• Group Dietary educational sessions
Diabetes specialist services clinics are provided at Wembley Centre for Health and Care and the Willesden Centre for Health and Care
